Collect document

  • Please prepare the following documents:
    • Death certificate (original or copy certified by competent authorities)/ death notice;
    • Death statement related to the life assured’s death cause;
    • Evidences to prove the life assured’s death cause:
      • Death due to disease: hospital discharge note, medical summary/ copy of original medical record, other medical documents.
      • Death due to accident: investigation conclusion, accident report and forensic report
    • Proof of eligibility for insurance benefits: copy of ID card AND assignment note/ testament/ written agreement on appointing insurance benefit recipients in case of inheritance by law, with certification by relevant authorities.

  • Hard copy submission:
    • At FWD's Customer Service Counter;
    • By post to: FWD Vietnam, 11th Floor, Diamond Plaza, 34 Le Duan, Ben Nghe Ward, District 1, HCMC.

Time limit for claim submission

Please submit claim documents within 12 months from the event date.

Remarks:

  • Depending on each specific case, FWD reserves the right to request the claimant to provide other supplementary document or original document for accurate claim assessment, legally.
